- Eligibility: Employee must have completed one full year of employment.
- Benefits: Provides job-protected, unpaid leave for up to 12 weeks.
- To Apply: Contact Elissa Puentes (HR) to initiate the FMLA process.

If Extended Leave is Required:
- Inform Supervisor/Principal promptly.
- Submit Doctor's Documentation to your supervisor/principal.
- The supervisor must send the documentation to Elissa Puentes (HR) within 24 hours.

- The employee must have their physician complete the Return to Work Form.
- The employee will submit the completed form to Elissa Puentes in HR.
- Return to work is contingent on HR clearance based on the doctor’s release and job compatibility.
- If the employee cannot resume duties, HR will provide information on additional leave options, including FMLA or Sick Leave Bank.

All employees are required to complete the following modules:
- Bloodborne Pathogen Exposure Prevention
- Child Abuse: Mandatory Reporting
- FERPA: Confidentiality of Records
- Sexual Harassment: Staff-to-Staff

- Eligibility: Available upon employment or during open enrollment periods.
- Enrollment: Application must be submitted before the deadline; late submissions will not be accepted.
- Application Process:
- Obtain the Sick Leave Bank application from Yvonne Acosta at yvonne.acosta@carlsbadschools.net
- Application will be reviewed by the Sick Leave Bank Committee.
- If approved:
- Up to 10 days of leave will be granted initially.
- Approval of the sick leave bank must be sought before the employee returns to work.
- Sick leave bank days are not retroactive.
- For additional days (up to 30 per school year), the employee must apply for an additional 10 days.
- Once a return to work has been approved, the remaining sick leave bank days go back into the sick leave bank.
- Any future days will be docked at the employees daily rate.

- When to Use: If an employee is injured on the job.
- Steps:
- Complete the Accident Report Form/PROPA. This form can be obtained from your supervisor. Notice of Accident Report Form/PROPA
- Submit the form to your supervisor/principal.
- The supervisor must send the Accident Report Form/Witness Statement Form/Supervisor Statement Form, and any documentation to Elissa Puentes (HR) within 24 hours. The forms must be completed; otherwise will be sent back to the supervisor.
- IF medical attention was sought, HR will contact the employee with a claim number and further details.
In order to return to work, the employee must submit the PROPA (Physical Report of Physical Ability) to Elissa Puentes. Return to work is contingent on HR clearance based on the doctor’s release and job compatibility.
